Getting
Started
All
you need is a referral from your physician to participate
in Medcentra’s outpatient rehabilitation program.
For your convenience, we’ve included a Physician’s
Referral form for you to print and share with your physician.
After
we receive a referral and some basic health information
from your physician, we will contact you to set up your
initial appointment.
Insurance
We will review your coverage before your first visit and
help you understand your insurance benefits. Medcentra will
handle all the details with your insurance company. We accept
Medicare, Medical Assistance, and most private insurance
plans.
